Grill Fumes and the Skin Despite its popularity, outdoor grilling comes with many dangers. Many people are aware that inhaling the fumes from a barbecue is far from ideal, and many of us will also know that eating grilled food can have negative consequences for our health. However, according to a new study, skin absorption of PAH could be an equally important issue — and one that’s mostly been overlooked. Researchers at Jinan University in China, led by Eddy Y. Zeng, set out to quantify exactly how much PAH passes through someone’s skin when they grill. Their results were recently published in the journal Environmental Science & Technology. To study this, the researchers attended a barbecue in Guangzhou, Guangdong Province, China. They divided 20 revelers into three groups: the first was exposed to fumes, food, and skin contact; the second was only exposed to fumes and skin contact; and those in the third were only exposed to skin contact (they wore hoods and masks and breathed compressed air throughout the event). Participants provided four urine samples: 17 hours prior to the barbecue, on the morning of the event, immediately prior to the start, and 35 hours after the event.

Studies show that young people who see smoking in movies are more likely to start smoking.

A more recent influence on tobacco use is the e-cigarette and other high-tech, fashionable electronic “vaping” devices. Often mistaken for harmless, these devices are easier to obtain and use than traditional tobacco products. They are a way for new users to learn how to inhale and become addicted to nicotine, which can prepare them for smoking.

How nicotine affects you

Nicotine and other chemicals in tobacco smoke are easily absorbed into the blood through the lungs. From there, nicotine quickly spreads throughout the body.

Taken in small amounts, nicotine causes pleasurable feelings and distracts the user from unpleasant feelings. This makes the tobacco user want to consume more. It affects the chemistry of the brain and central nervous system and affects mood. Nicotine works similarly to other addictive substances by flooding the brain’s reward circuitry with a chemical called dopamine. Nicotine also gives a small rush of adrenaline – not enough to notice, but enough to quicken the heart and raise blood pressure.

Nicotine reaches the brain within seconds of a hit and wears off within minutes. The user may begin to feel irritable and nervous. Over time, a certain level of nicotine is reached and the individual must maintain consumption to keep nicotine levels within a comfortable range.

People who smoke can quickly become addicted to nicotine and experience physical and emotional (mental or psychological) withdrawal symptoms when they stop smoking. These symptoms include irritability, nervousness, headaches, and trouble sleeping. The true hallmark of addiction, however, is that people still smoke despite knowing that smoking is bad for them — and affecting their lives, health, and families in unhealthy ways. In fact, most smokers want to quit.

Researchers are also studying other chemicals in tobacco that make it harder to quit. Tobacco smoke causes chemical changes in the brains of animals that cannot be fully explained by the effects of nicotine.

The average amount of nicotine in a regular cigarette is around 1 to 2 milligrams (mg). The amount you actually take in depends on how you smoke, how many puffs you take, how deeply you inhale, and other factors.

How strong is nicotine addiction?

About 2 in 3 smokers say they want to quit and about half try each year, but few manage without help. Because they don’t just become physically dependent on nicotine. There is also a strong emotional (psychological) dependence. Nicotine affects behavior, mood and emotions. People who inhale cigar smoke absorb nicotine through their lungs just as quickly as people who smoke cigarettes. For those who do not inhale, the nicotine is absorbed more slowly through the lining of the mouth. This means that people who smoke cigars can get the desired dose of nicotine without inhaling the smoke directly into their lungs.

Most large cigars have as much nicotine as several cigarettes. Cigarettes contain an average of about 8 milligrams (mg) of nicotine, but only deliver about 1 to 2 mg of nicotine. Many popular brands of larger cigars contain between 100 and 200mg or even up to 444mg of nicotine. The amount of nicotine that a cigar delivers to a smoker can vary greatly, even when smoking the same type of cigar. How much nicotine is absorbed depends, among other things, on the following factors:

How long the person smokes the cigar

How many moves are taken

Whether the smoke is inhaled

Given these factors and the wide range of cigar sizes to choose from, it’s almost impossible to give a good estimate of the amounts of nicotine that larger cigars will deliver.

Small cigars, which are the size and shape of cigarettes, have about the same amount of nicotine as one cigarette. When these are smoked (inhaled) like cigarettes, they are expected to deliver a similar amount of nicotine – 1 to 2mg.

Nicotine in smokeless tobacco

Smokeless tobacco delivers a high dose of nicotine. Nicotine enters the bloodstream through your mouth or nose and is carried to every part of your body.

Nicotine in smokeless tobacco is measured in milligrams (mg) of nicotine per gram (g) of tobacco. It has been found to vary widely, for example up to 4 to 25 mg/g for wet snuff, 11 to 25 mg/g for dry snuff and 3 to 40 mg/g for chewing tobacco. Other factors that affect the amount of nicotine a person gets are things like:

However, blood nicotine levels have been shown to be fairly similar when comparing people who smoke cigarettes to those who use smokeless tobacco.

Nicotine in unburned products

Unburned tobacco products come in a variety of forms and are used in a variety of ways. Unburned products contain nicotine and can lead to nicotine addiction.

Heat-not-burn cigarettes have a heat source and tobacco. The tobacco is heated to a lower temperature than a normal (combustible) cigarette. The heat creates an aerosol that is inhaled by the user.

Dissolvable tobacco products are edible. They can be lozenges, strips, gummies or sticks. They can be easily hidden and look like candy.

Nicotine gels are tobacco products that are rubbed onto and absorbed by the skin.

Nicotine in e-cigarettes

The e-liquid in most e-cigarettes (vapes) contains nicotine. However, the nicotine content is not the same for all types of e-cigarettes and sometimes product labels do not state the true nicotine content.

There are some e-cigarette brands that claim to be nicotine-free but have been found to contain nicotine.

Why is it so hard to quit tobacco?

Quitting or reducing tobacco use causes symptoms of nicotine withdrawal. Withdrawal is both physical and mental. Physically, your body reacts to the lack of nicotine. Mentally, you are faced with breaking a habit that requires a fundamental change in behavior. Emotionally, you might feel like you lost your best friend. Studies have shown that smokeless tobacco users have as much trouble quitting as people trying to quit cigarettes.

People who have been using tobacco regularly for a few weeks or more will experience withdrawal symptoms if they suddenly stop or greatly reduce the amount they use. There is no danger with nicotine withdrawal, but the symptoms can be uncomfortable. They usually start within a few hours and peak about 2 to 3 days later when most of the nicotine and its by-products have been eliminated from the body. Withdrawal symptoms can last from a few days to several weeks. They are getting better every day that a person stays tobacco free.

Nicotine withdrawal symptoms can be any of the following:

Although the effects of smoking are well known, smokers often assume that smoking just a few cigarettes or smoking without inhaling is not hazardous. In this study smoking without inhaling was associated with a significantly increased risk of myocardial infarction and all cause mortality.

Although the dangers of smoking are well described, it is often assumed among smokers that consuming a few cigarettes or smoking without inhaling the smoke is safe. From both a health and clinical perspective, it is important to recognize the health risks associated with low consumption in order to provide appropriate smoking advice.

The risk of myocardial infarction (MI) and all causes of death increases with increasing tobacco use, but most previous studies have used a cut-off value of 10, 15, or 20 cigarettes per day.1–3 Determining the dangers of light smoking with sufficient accuracy requires large population studies and few studies have attempted this.4–6 To our knowledge, no study has described the risk associated with non-inhalation smoking.

The Copenhagen City Heart Study has followed up 14,223 people with an unusually high proportion of smokers, both males and females, since 1976. By the end of 1997, 5188 deaths and 1348 cases of MI had been registered and the study now offers the opportunity to examine the effects of light and non-smoke smoking on the risk of developing MI and on all-cause mortality.

METHODS Population and design We used data from the City of Copenhagen Heart Study, which was previously detailed.7 In summary, the population was derived from a random, age-stratified sample of 19,698 individuals aged 20 years or more, recruited from 90,000 individuals living in a specific area in Copenhagen. A total of 14,223 subjects (response rate 72%) took part in the first test in 1976/78. Subjects were followed through December 31, 1997 for fatal and non-fatal MI (ICD-8 diagnostic code 410 through January 1, 1994, ICD-10 diagnostic codes I21 and I22 as of 1994) assessed by the National Board of Health and the National Hospital discharge registries or patients with self-reported and verified ischemic heart disease (IHD) prior to enrollment were excluded and therefore the analyzes relate to the first MI only. In addition, only subjects over 30 were included to ensure smoking habits were established at baseline. All subjects with sufficient information on cardiovascular risk factors were included. Variables of interest Cardiovascular risk factors were assessed using a self-administered questionnaire and various laboratory tests. Participants reported smoking status as never-smokers, ex-smokers, or current smokers. Current smokers reported the amount and type (cigarette, cheroot, cigar, pipe, or mixed) of tobacco currently smoked and whether he or she inhaled. Current tobacco use was calculated by equating a cigarette to 1 gram, a cigar to 3 grams, and a cigar to 5 grams of tobacco. Arterial blood pressure was measured in a sitting position after at least five minutes of rest and analyzed into five categories: systolic blood pressure (SBP) < 140 and diastolic blood pressure (DBP) < 90; 140 ≤ SBP < 160 or 90 ≤ DBP < 100; 160 ≤ SBP < 180 or 100 ≤ DBP < 110; 180 ≤ SBP or 110 ≤ DBP, and; Taking medication for high blood pressure. Blood lipids were not measured on an empty stomach. Total cholesterol was divided into three categories: less than 5; 5-7 and; 7 mmol/L or more. Triglycerides were also divided into three categories: less than 1; 1-3 and; 3 mmol/L or more. Body mass index (BMI) was calculated as weight (kg) divided by height squared (m2) and analyzed into four categories: less than 20; 20-25; 25-30 and; 30kg/m2 or more. Two socioeconomic status variables were included: level of education, which was divided into three categories: less than eight years of schooling (completed elementary school); 8-10 years and; more than 10 years and household incomes categorized as low, medium, or high income. Alcohol consumption was categorized by daily intake into: less than one drink per day; 1-2; 3-4; and more than 4 drinks per day, one drink with 9-13 g of alcohol. Recreational physical activity was classified as sedentary; moderate activity less than four hours; and moderate activity of more than four hours per week. Family history of diabetes and MI were also self-reported. Statistical Analyzes To assess the independent contribution of smoking to MI and all-cause mortality, the Cox proportional hazards model 8 was used with age as the underlying time scale and delayed onset accordingly. The analyzes were performed separately by gender and adjusted for confounders as indicated. Using age as the underlying time scale ensured optimal age adjustment. All covariates were treated as categorical variables as described above and interaction tests were performed using the likelihood-ratio test. The proportional hazards assumption was tested by interaction terms with age using the likelihood-ratio test. All results presented were adjusted for the cardiovascular risk factors listed in Table 1. After five years, 11,094 subjects were re-examined. Repeated analyzes using the updated smoking habits whenever possible did not affect the risk estimates and the results shown are therefore based on the smoking habits at baseline. RESULTS After excluding 1207 women and 867 men for age under 30 (n=610), pre-entry IHD (n=298), or missing data on one or more variables (n=1166), 6505 women and 5644 men were for analyzes available. For each covariate, fewer than about 100 subjects of both sexes were missing data, with the exception of household income, which 455 women did not report. Only 37 men and 36 women did not indicate their smoking habits. During follow-up, 476 women and 872 men suffered a heart attack, of which 40% were fatal (182 women, 362 men). A total of 2305 women and 2883 men died from all causes. Baseline characteristics of participants at study entry with an emphasis on cardiovascular risk factors are presented in Table 1. With the exception of age, men and women differed in all variables. Men had a more favorable risk factor profile for total cholesterol, alcohol consumption, physical activity, family history, household income, and education, while women had more favorable triglyceride levels, blood pressure, BMI, and history of diabetes. Details of the smoking properties are given in Table 2. There were more male smokers, they smoked more and the share of inhalers was larger. The number of male smokers with low tobacco use was limited, especially the number of non-inhalers. The type of tobacco smoked differed by gender, with 81.2% of women and 56.8% of men being cigarette smokers. The distribution of smoking habits among the 1166 subjects excluded due to missing data did not differ from the smoking habits of the rest. In both men and women, the risks were significantly higher in smokers who claimed to inhale than in smokers who did not. The overall effect of inhalation was 1.65 (1.28 to 2.13) in women and 1.39 (1.14 to 1.70) in men. In women, significantly increased risks were found for consuming 6 grams or more of tobacco per day in non-inhalers and 3 grams or more in inhalers. In male smokers who inhaled, the risks were significantly increased from a consumption of 6 grams of tobacco per day. Risks were not significantly increased in any of the non-inhaler categories in males, and the linear trend test was also insignificant (p=0.19). Results from Cox proportional hazard regression analyzes RR related to smoking were higher in women than in men. In a model that adjusted for amount smoked (three categories), inhalation, and type of tobacco in addition to cardiovascular risk factors listed in Table 1, the interaction between gender and smoking was highly significant, with a higher rate in female smokers compared to males relative risk was found smokers. Blood pressure, total cholesterol, triglycerides, education, household income, alcohol consumption, recreational physical activity, MI, and family history of diabetes, but not BMI, were strongly associated with MI risk.
