My perception of the difference between the two as commonly used is that “determine” can also be used to describe something that has been discovered, whereas “decide” isn’t used the same way. So if there has been an accident you could have investigators researching what the cause of the accident was, and after they are finished “they determined that the cause of the accident was [whatever]”. I don’t think the word “decide” would be used that way. An investigation is often supposed to be neutral, and the word “decide” to me implies a more “personal” or “subjective” choice. So for example:

After it was determined that the accident was caused by faulty brakes on the car it was decided it should be recalled.

The neutral investigation determined the problem. There was then a discussion about what to do about it and a decision was made.
