Have a Healthier Winter With San Fu Moxa Now!

Many of our patients are familiar with moxibustion: burning mugwort on acupuncture points to dispel cold, alleviate pain, and even turn breech babies. But once a year, we use something called San Fu Moxa.

San Fu loosely translates to heavenly or celestial moxibustion. Every year during the “dog days” of summer, there is a tradition of applying a paste of warming herbs to points on the back to support immunity in the coming winter. Preventing illness is a cornerstone of Chinese Medicine. Here, we’re preventing Winter disease by treating you in the summertime.

We call this San Fu Moxibustion. Instead of mugwort, we grind warming herbs and apply them as a paste to points on the back associated with the lungs. This year caught me off guard, and we missed the first day, but it’s not too late. Even 1-2 treatments can support lung health. It’s a traditional treatment for the prevention of allergic rhinitis, bronchitis, and asthma. This tradition has never been more critical than right now! This traditional treatment has been used since the Qing Dynasty, the warmth of the herbs penetrates the body. San Fu refers to three 10-day periods that are predicted to be the year’s hottest days – what we call “the dog days of summer.”

We grind and sift the herbs, and then we combine them with water or ginger juice to form a paste, which is secured on the back with a bandage. The herbs can be left on the skin for several hours. In younger children, 20-30 min hours will suffice. We generally don’t use this on anyone under 2 years old.

Some of the traditional herbs used are difficult to procure now, so I’ve created my own blend.

We are looking to create a blister or skin reaction. This is normal and is a positive response. It helps the herbs penetrate the body. Everyone has a different reaction. Some common responses you can expect:

localized redness, itching or blistering where the plaster is applied

feverishness or flu-like symptoms that can last up to 24 hours

It’s common to have a red dot that resembles sunburn for a few hours afterward. Blistering can take up to 3 days to emerge and may not even occur.

Here’s a picture of my back from last summer. I had left the Moxa on for about 6 hours and I developed itching and redness that abated after 2-3 days.

Home care includes:

Remove the San Fu applications after 6 hours or if they start to become overly hot and itchy – whichever comes first.

Clean the skin with soap and water and gently pat the skin dry with a clean cloth or towel.

If blisters appear, treat them as you would with any normal blister. Keep the area clean, and once the fluid comes out, do not remove the top layer of skin. Keep the wound clean at all times and cover with a bandage during the day to keep it protected. Feel free to use an antibiotic or a topical ointment such as bacitracin or Neosporin. Summer’s Here; Time for San Fu Moxa

Here’s the 2022 dates: on July 16th, 26st, August 5th, and 15th. Acupuncturists around the globe use San Fu Moxa to help prevent respiratory illness for the following winter. Thanks to Heidi Lovie for calculating the dates.

The days are selected on the most cosmologically auspicious days of the year: a series of days that begin with a yang metal stem. The intervals are somewhat irregular, ranging from 10-20 days. The final treatment is added after the first day of Autumn, according to the Chinese Calendar.

Since the Qing Dynasty, San Fu has been used to help prevent asthma, bronchitis, and other respiratory disorders. Commonly we think of moxa as burning mugwort on points or areas of the body. However, San Fu Moxa is considered a cold moxa: nothing is burned, but the herbs are warming.

Why do it now?

Because we honor the principles of prevention. In this case, Dong Xia Bing Zhi

“Treat winter diseases in the summer.”

Chapter 9 of the Ling Shu says that “In spring Qi is in the hair, in summer Qi is in the skin, in Autumn Qi is in the flesh, and in winter Qi is in the tendons and bones. While this passage was discussing needling depth, we can apply this principle to San Fu Treatment.

The lungs rule the skin and belong to the metal element. Therefore, treating the points during the peak hot days of the year allows the warmth of the herbs to penetrate, warming the yang, transforming phlegm, and strengthening the lungs.

How to use San Fu Moxa

The following is how we use San Fu Moxa. Other practitioners might select different points. If you have your own method, please share this with us! We love learning from our community.

Our San Fu uses a mixture of equal parts Xi Xin Radix et Rhizoma Asari, Bai Jie Zi, Semen Sinapis, Yan Hu Suo Rhizoma Corydalis, Lai fu Zi Semen Raphani, Xuan Fu Hua Flos Inulae, and Wu Zhu Yu Fructus Evodiae. Traditionally, Gan Sui Radix Kansuiwas included, as was She Xiang Moschus. However, the former is difficult to source, and the latter is illegal and unethical. Ma Huang (Ephedra Herba) can also be used; however, its restricted use makes this difficult in the U.S.

Each bag contains 25 grams each for a total of 150 grams.

The herbs are ground and sifted into a fine powder and are ready to use when mixed with the moistening agent of your choice.

Moisten the herb powder with the liquid of your choosing: we like water or ginger juice. Other options include honey, sesame oil, and even petroleum jelly.

They are formed into small balls (we measure ¼ teaspoon) and applied to a series of changing points.

Here’s the point selection for each treatment

The first treatment focused on downbearing and diffusing the lungs:

Dingchuan, BL‑12, BL‑13

The second treatment supports the spleen and transforms phlegm:

DU‑14, BL‑14, BL‑20

The Final treatment nourishes the kidneys to aid them in grasping the lung qi

BL‑11, BL‑23, BL‑43

The herbs are applied and left on for several hours or up to a day. They frequently feel itchy, and the skin reddens. Blisters forming is possible. If this happens, instruct your patients to apply a soothing burn cream. If they come to the office, you can apply Ching Wang Hung Ointment.

The length of time the San Fu remains on the skin can vary widely: it can feel caustic and itchy after an hour. On the other hand, in the case of a yang deficient patient last year, she never felt the heat from the application at all. We generally instruct our patients to remove the plaster when it feels irritating.

Consider San Fu for asthmatic children: make the plasters smaller: pea-sized and retain them only for a couple of hours.

The treatment may leave marks that fade over time: Make your patient aware of this in advance.

San Fu Moxibustion is a treatment given during the hottest days of summer. It is a therapy where we “treat winter diseases during the summer”. For centuries, people have been given this warming herbal plaster treatment 3 times over the course of one month spanning the dog days of summer. The concept behind San Fu is that we are capturing some of the strongest yang energy of the year and “hiding it away” to warm our immunity and prepare for autumn when cold & flu, allergies and respiratory illness run rampant.

One cornerstone of Asian medicine is the element of preventative medicine. In the wake of our recent pandemic, it is increasingly vital that we keep our immune systems at peak performance. This therapy has been clinically researched to do just that!

The Process

Typically, the first treatment is given on a specified day in July. The second is given 10 days later and the third, 10 days after. The exact dates are calculated according to the Chinese Lunar calendar as the 3 predicted hottest days of the year. At this time you can effectively treat the ailments of the winter and support health by nourishing the yang energy of the body. San Fu essentially prevents illness and provides resilience against the cold pathogens of the winter season. The San Fu treatment uses acrid, warm and dispersing herbs that are ground into powder and mixed with ginger juice. Once mixed they form an herbal paste. The paste is applied to specific points on the body, usually on the upper back. These points correlate with bolstering the immune system, fortifying the lung and dispersing “wind” or exterior pathogens. The plaster is left on the points for up to 6 hours. The herbs are very warm and may increase in intensity to produce local redness, heat, and/or even blisters. This is normal and with proper hygiene & care, heal very well. The suggested dosage of treatment is to receive the 3 consecutive treatments during these specific summer dates for 3 summers consecutively.

SAN FU MOXIBUSTION— PREPARING FOR THE FALL AND WINTER SEASONS

With many people wondering whether or not there will be a COVID spike this coming fall, it is even more crucial that your patients’ immune systems are functioning optimally and, since preventative measures is one of the pillars of Traditional Chinese Medicine, you have the ability to “treat winter diseases in summer” with San Fu Moxa— a traditional treatment that has been administered as far back as the Qing Dynasty to support immune function and has been clinically researched to be effective.

Also known as “heavenly” or “celestial” moxibustion, San Fu Moxa is offered during the three 10-day periods that are said to be the hottest days of the year according to the Chinese Lunar calendar or “the dog days of summer.” Warming herbs that have been ground into a paste, usually with fresh ginger juice, are rolled into a ball, flattened, and then applied to acupuncture points on the back indicated for lung health. They are then secured with medical tape in order to bolster immunity in anticipation of the winter season.

During the San Fu period, the pores or “Defensive Qi” are open and receptive to expelling external cold pathogenic factors, thereby harnessing the strongest yang energy of the year and storing it for future use in preparation for autumn and winter respiratory ailments. The herbs can be left on the back for up to six hours and seeing blistering or some sort of heat/redness where the herbs were applied is common, as it indicates a successful treatment and that the herbs have entered the body. Such a reaction on the body is preferable and should not be a cause for concern. Any marks should not be permanent and will fade eventually. A warming or tingling sensation is appropriate, but if the patient feels any burning or pain in the applied area, the paste should be removed. If at any point the area feels irritated, the paste should be removed. If you have patients who have limited sensation you should use caution or refrain from this method altogether as they may not be able to adequately gauge the sensations occurring through the treatment.

– 2022 July 16, July 26, August 5, August 15

Even though two of the days have already passed for this year, there are still two more remaining and still one or two treatments can be effective and improve lung health—preventing chronic respiratory conditions such as bronchitis, asthma, chronic coughs or colds, emphysema, and others in addition to helping those who are immune compromised or have an overall weak/susceptible constitution. Chinese Medicine says that after three consecutive years of San Fu treatment, these conditions will be fully cleared in the patient.

San Fu Moxa Summer Immunity Treatments: July 11, July 21, July 31, August 10 (Online)

San Fu Moxa is an herbal paste that is applied to acupuncture points with on the most Yang days of the summer in order to boost immunity and build yang in the body come the winter months. This is recommended for those who deal with allergies, asthma, frequent colds, arthritis and low immune conditions.

For this workshop you will receive a packet of herbs beforehand. An online zoom class will be given on each date to give guidance on how to apply the herbs. The herbal paste will need to be made with ginger juice. It should be worn for 3-6 hours and is applied with waterproof medical tape or tegederm (included in your packet). *The herbal paste can cause skin irritation, rash and scarring. Applying treatment 3 times during the Summer will make the series more effective.

During the workshop you will learn about the herbal ingredients, how to prepare it, and how to safely apply it.
