The round window is one of the two openings from the middle ear into the inner ear. It is closed by the secondary eardrum (round window membrane), which vibrates out of phase with vibrations entering the inner ear through the oval window. It allows fluid to move in the cochlea, which in turn stimulates the hair cells of the basilar membrane and allows hearing to take place.

Structure[edit]

The round window is below (below) and slightly behind (behind) the oval window, from which it is separated by a rounded elevation, the promontory.

It is located at the base of a funnel-shaped depression (the round window niche) and opens into the macerated bone in the cochlea of ​​the inner ear; when fresh it is closed by a membrane, the secondary tympanic membrane (Latin: membrana tympani secundaria or membrana fenestrae cochleae) or round window membrane, which has a complex saddle-point shape. The visible central portion is concave (curved inwards) towards the tympanic cavity and convex (curved outwards) towards the cochlea; but towards the edges, where it is tucked away in the round window niche, it curves the other way.

Both the oval and round windows are approximately the same size, approximately 2.5 square millimeters (0.0039 square inches). The entrance to the round window niche is often much smaller than this.

Function[edit]

The stirrup transfers the movement to the oval window. When the footplate of the stapes moves into the oval window, the membrane of the round window moves out and this allows movement of fluid in the cochlea, resulting in movement of the inner hair cells of the cochlea and hence hearing. If the round window were absent or rigidly attached (as can occur with some congenital anomalies), the footplate of the stapes would force incompressible fluid against the unyielding walls of the cochlea. It would therefore not move to a meaningful extent, resulting in a hearing loss of around 60 dB. Not surprisingly, this is the same as conditions where the stapes itself is fixed, such as: B. Otosclerosis.

The round window is located in the mesotympanum at the posterior end of the basal convolution of the cochlea. The oval window is also in the mesotympanum and opens at the inferior and lateral part of the vestibule.[1] Both are clearly visible on CT.

Clinical significance[ edit ]

The round window sometimes does not develop properly, causing the hearing loss mentioned above. Unfortunately, round window malformations are often associated with other ear malformations and the hearing loss can be much more serious. Some types of ear surgery (now generally abandoned) left the round window open to the outside world and covered the oval window. The sound pressure therefore hit the round window but was shielded by the oval window. It therefore traveled “backwards” around the cochlea but still gave usable hearing as the hair cells were still deflected in the same way. The round window is often used as a portal for cochlear implant surgery. It has also recently been used as a site to place middle ear implantable hearing aid transducers.
